Job summary

A&T Moore Health Care Services is seeking a compassionate Care Coordinator to join our growing Arlington team. You will coordinate client intakes, schedule caregivers, and manage ongoing care needs to ensure high-quality home care.

In this on-site role, you’ll serve on the on-call rotation, triage urgent shifts after hours, assist with hiring, and help promote referral opportunities while supporting our caregivers.

Qualifications

• 1 year home care or home health care office experience preferred.
• Strong phone and communication skills, friendly, reliable, and detail-oriented
• Reliable transportation, valid driver’s license, and comfort using mobile apps / EMR systems
• Strong attention to detail and multitasking ability.
• Respond to incoming calls, inquiries, and service requests promptly
• Ability to lift / transfer clients and adhere to all health and safety guidelines
• Willingness to participate in an on-call rotation

Responsibilities

• Conduct on-site visits and introductions
