Job summary

A & T Moore Health Care Services in Arlington, TX is seeking a motivated, organized, and compassionate Care Coordinator to join our growing team. You will coordinate new client intakes, caregiver scheduling, and ongoing care needs with a focus on high-quality, compliant service.

You’ll participate in an on-call rotation, triage caregiver call-outs, fill urgent shifts, and help identify referral opportunities while supporting our hiring process.

Qualifications

• 1 year home care or home health care office experience preferred.
• Strong phone and communication skills, friendly, reliable, and detail-oriented
• Reliable transportation, valid driver’s license, and comfort using mobile apps / EMR systems
• Strong attention to detail and multitasking ability.
• Respond to incoming calls, inquiries, and service requests promptly
• Ability to lift / transfer clients and adhere to all health and safety guidelines
• Willingness to participate in an on-call rotation

Responsibilities

• Marketing Opportunities: Identify referral opportunities and assist with marketing initiatives.

• Support our caregivers and ensure quality follow-up
• Monitor client satisfaction and follow up on any issues or concerns compliance with care plans.
• Conduct on-site visits and introductions
