Visiting Angels of Santa Barbara and Santa Maria is seeking a skilled Care Coordinator to join our homecare team in Carpinteria, CA. You will collaborate with caregivers, physicians, and families to determine client medical needs and oversee employment to ensure high-quality, individualized care.

The ideal candidate is compassionate, reliable, and knowledgeable about homecare practices, with strong problem-solving, organizational, and communication skills to manage multiple client plans

Qualifications

• Experience as a Care Coordinator or similar role preferred.
• Certification as a medical assistant preferred.
• Strong problem-solving and organizational skills.
• Ability to manage multiple projects or tasks and prioritize appropriately.
• Ability to work in fast-paced situations and make sound decisions quickly.
• Excellent interpersonal skills and high level of compassion.
• Strong verbal and written communication skills.

Responsibilities

• Collaborate with physicians, patients, families, and healthcare staff.
• Review care plan to ensure it is being followed; update/train as needed.
• Develop individualized care plans.
• Accompany clients to doctor/hospital visits.
• Create goals and monitor progress toward goals.
• Recruit and train staff.
• Create schedules.
• Manage 20-40 clients.
• After-hours phone.
