Qualifications

• Master’s degree in Social Work (MSW) or related field required.
• Strong clinical and communication skills.
• Experience in hospital or acute care settings preferred.

Responsibilities

• Member of a collaborative practice model with patients, nurses, care coordinators, physicians and the community.
• Provides therapeutic interventions through assessment, coordination, referral and interdisciplinary planning.
• Understands treatment, social and economic implications for individuals and families as they pertain to age, stage of development and discharge needs.
• Demonstrates skills in discharge and transition planning and linking with community resources.
• Exhibits planning, organization, and management of multiple functions and complex processes.
• Status: Full time, 40 hour week. Exempt.
• Weekend requirement: Every other
