Job summary

Virtua Health in New Jersey is seeking Per Diem Scheduling & Coverage RN/CM to coordinate patient care across five campuses. You will support both clinical and administrative processes, ensuring safe transitions along the care continuum.

Requirements include RN or MSW, 3+ years of relevant experience, and strong communication skills. Per Diem role requires weekday/weekend availability; on-site work with a comprehensive benefits package is offered.

Qualifications

• RN or Masters prepared Social Worker required.
• 3+ years clinical nursing or social work experience.
• 1 year case management or homecare coordination in an acute care setting.
• Licensure: RN or LSW; LCSW considered.

Responsibilities

• Care coordination with physician and multidisciplinary team to improve quality and cost effectiveness.
• Documentation of plan of care in EMR and case management systems.
• Meet objectives and dashboards contributing to organizational success.
• Participate in Six Sigma and other improvement activities.
• Identify and report compliance issues as appropriate.
