Job summary

Virtua Health in New Jersey seeks a part-time Per Diem Case Manager/Registered Nurse to coordinate patient care with physicians and multidisciplinary teams across five campuses, including Voorhees, Marlton, Camden, Willingboro, and Mt. Holly.

Ideal candidates have 3+ years nursing or social work, case management experience, and strong communication; Medicare/Medicaid knowledge is a plus. The role requires weekday and weekend availability 8:30 a.m.–5:00 p.m. and occasional holiday coverage.

Qualifications

• RN or Masters prepared Social Worker.
• Minimum 3 years clinical nursing (RN) experience or Social Work.
• Experience in Case Management or homecare coordination in an acute care setting; understanding of Medicare/Medicaid and discharge planning.

Responsibilities

• Care Coordination - assess and plan to meet patient needs, monitor length of stay, promote efficient resource use.
• Resolve problems and communicate with patients/families; refer Social Work as needed.
• Document plans of care in EMR and case management systems.
• Meet job-specific metrics and dashboards for organizational success.
• Participate in quality/continuous improvement activities and compliance.
