Job summary

Azalea Homecare in Georgia is seeking an organized Care Coordinator to manage client services, caregiver scheduling, staffing, and day-to-day communication. You will serve as the primary contact for clients, families, and caregivers, collaborating with operations, nursing, recruiting, onboarding, and billing in a fast-paced home care setting.

Qualifications

• Experience in home care scheduling, staffing, or client services.
• Strong communication and customer-service skills.
• Excellent organization with ability to manage multiple priorities.
• Comfortable using electronic scheduling and documentation systems.
• Experience with WellSky, EVV, Medicaid home care programs or private-pay care a plus.

Responsibilities

• Serve as primary contact for clients, families, and caregivers and communicate service needs clearly.
• Create and maintain caregiver schedules, coordinate assignments, and cover staffing gaps.
• Maintain client information, notes, schedules, and follow-up tasks in the system.
• Assist with onboarding and recruitment support to place caregivers with clients.
• Attend team huddles and provide updates on outstanding items and next steps.
