Job summary

Azalea Homecare Inc. in Lawrenceville, GA is seeking a Lead RN / Home Care Clinical Supervisor. This part-time role focuses on assessments, care planning, nursing oversight, and care coordination for home-based clients.

The position requires an active GA RN license, CPR/BLS, and strong clinical judgment. Travel to client homes is expected, with 20–30 hours per week and opportunities for advancement.

Qualifications

• Active, unrestricted RN license in Georgia.
• Graduate of an accredited nursing program.
• Current CPR/BLS certification.
• Strong clinical judgment, communication, organization and documentation skills.
• Ability to work independently and collaborate with an interdisciplinary team.
• Comfortable using electronic documentation systems and Microsoft Office.
• Reliable transportation and ability to travel to client homes.
• Availability for 20–30 hours per week.

Responsibilities

• Complete client acuity assessments within two business days of referral notification.
• Perform initial nursing assessments and home safety reviews.
• Develop, review, sign, and update individualized care plans.
• Identify fall risks, transfer needs, caregiver skill requirements, and condition changes.
• Conduct supervisory visits and reassessments.
• Communicate clinical concerns and care-plan changes with families and teams.
• Provide RN-level oversight of nursing staff and review documentation when required.
• Track assessments, supervisory visits and care-plan updates.
