You will handle referrals and authorizations, coordinate care for preventative services, engage with IPA/HMO payers, and ensure timely communication and documentation while traveling to cover multiple clinics.

This position will report to the Area Manager and will be primarily clinic-based. There may be some travel to other health clinics. Candidates should reside in the Natomas/Sacramento area

The Care Coordinator is responsible for managing and coordinating patient care to ensure optimal patient outcomes. This role involves working closely with patients, healthcare providers, and other external payer stakeholders to facilitate access to necessary services, and provide continuous support throughout the healthcare journey.

WHAT YOU’LL DO

• Referrals and Authorizations Coordination

The pay range for this role is:
