Job summary

Centene is seeking a Care Manager to develop, assess and coordinate care plans for members, educating them and families on services and benefits. The role requires a bachelor’s degree and 2–4 years of related experience, with nursing licensure if applicable, and allows flexible schedules including remote or hybrid work.

Qualifications

• Requires a Bachelor’s degree and 2–4 years of related experience.
• Graduate from an Accredited School of Nursing if holding clinical licensure.

Responsibilities

• Evaluates member needs, barriers to care, resources, and recommends plans for best outcomes.
• Develops or contributes to personalized care/service plans and identifies providers and community resources.
• Provides psychosocial and resource support to members, caregivers, and care managers to access local resources.
• Coordinates between member/family and care provider team to ensure timely access to services.
• Monitors progress toward care plan goals and collaborates with providers for revisions.
• Collects and maintains all member information to ensure regulatory compliance.
• May perform on-site visits to assess member needs with providers or resources.
• Educates care managers and members on procedures, instructions, options, referrals and benefits.
• Performs other duties as assigned.
