Traditional Home Care is seeking a Care Coordinator in Reading, PA to monitor and support daily client services in compliance with state regulations and company policy. You will coordinate care, manage scheduling, and ensure quality service for clients living at home.

The role focuses on building relationships with clients and referral sources, hiring and orienting caregivers, and supporting EVV and payroll functions to maintain seamless operations.

Qualifications

• Previous home health care or home care experience highly desirable.
• High School or GED required. Associate's Degree preferred.
• Staffing experience preferred.
• Excellent verbal/written communication, time management and organizational skills.
• Ability to effectively multi-task.
• Effective problem solving skills.
• Strong presentation and training skills.
• Strong customer service skills.

Responsibilities

• Provide superior customer service and quality home care.
• Build lasting relationships with clients, referral sources, payers, and community organizations.
• Coordinate client services and manage scheduling issues.
• Manage and grow your client caseload.
• Develop strong, communicative relationships with the team.
• Interview, hire, and orient reliable and compassionate caregivers.
• Function as primary support for daily EVV and payroll functions.
