Job summary

Pathway Society in San Jose, CA is hiring a Care Coordinator to serve as a liaison between clients, families, medical and mental health practitioners, and criminal justice agencies. The role involves educating clients on daily living activities and treatment opportunities to ensure a positive treatment experience.

Qualifications

• One-year practical experience in drug/alcohol or mental health field preferred; RADT certification in progress encouraged
• Excellent written and verbal communication skills
• Commitment to high ethical standards per Pathway employee handbook
• Client-focused attitude with professional demeanor and problem-solving ability
• Comfort with frequent interruptions in a fast-paced, client-centered environment
• Ability to respond physically to client needs, including stairs and facility navigation
• Maintain appropriate personal and professional boundaries with clients and co-workers
• Ability to pass comprehensive background and reference checks; may require driver's license and insurance

Responsibilities

• Assist with intakes (safety checks, COVID testing, etc)
• Conduct regular rounds of the facility and daily dorm inspections, ensuring client safety and participation
• Assist with medication monitoring, counting, disposal, documentation in Welligent
• Perform urinalysis testing
• Educate and remind clients of facility rules, policies and procedures
• Refer clients to counselors for clinical support
• Document client movement and provide transportation resources for appointments; accompany clients off-site when needed
• Maintain continuity among work teams by communicating actions and ongoing client needs
• Document movements for medically urgent appointments and provide support as needed
• Perform additional duties as assigned by program director
