Job summary

Pathway Society in San Jose, CA seeks a Care Coordinator to liaison between clients, families, medical and mental health practitioners, and criminal justice agencies. You will educate clients on daily living and treatment opportunities, support intakes, and monitor medication and safety throughout the program.

The role demands teamwork, clear communication, and the ability to adapt in a fast-paced environment while upholding professional ethics. A background in clinical settings is preferred.

Qualifications

• One-year practical experience in drug/alcohol or mental health field preferred.
• Excellent written and verbal communication skills and demonstrated ability to work effectively within a team as well as alone.
• Must be comfortable with frequent interruptions in a fast paced, intensive, client centered environment.
• Must pass comprehensive criminal background check; valid driver's license and insurance preferred.

Responsibilities

• Assist with intakes (safety checks, COVID testing, etc).
• Conduct regular rounds of the facility, daily dorm inspection, and ensure client safety and participation in activities.
• Assist with medication monitoring, counting, disposal, documentation in Welligent, and compliance as needed.
• Perform urinalysis testing.
• Educate and remind clients of the rules, policies and procedures of the facility.
• Refer clients to their counselor for clinical support.
• Document client movement in and out of the facility; provide transportation resources to appointments and accompany clients to off-site visits.
• Maintain continuity among work teams by documenting actions and ongoing client needs.
• Perform additional duties as assigned by program director.
