Phoenix Children's Hospital, Inc. is seeking a Resource Coordinator in the Care Management group to support Social Work, Case Management and Utilization Management.

The role focuses on administrative duties that promote a positive patient-family experience, facilitate safe and timely discharge, and reduce unfunded services and costs during hospitalization. The position involves direct interaction with patients, families, providers, and payers, under regular supervision by the Manager, Case

Qualifications

• High school diploma or equivalent required.
• Associates or Bachelors in related field preferred.
• At least three years of health care experience (nursing student, medical assistant, home care coordinator, etc.).
• Experience with medical terminology preferred.
• Excellent communication skills, both verbal and written.
• Effective critical thinking and ability to anticipate discharge needs.
• Moderate computer skills.
• Strong decision-making and problem-solving abilities.
• Bilingual in Spanish preferred.

Responsibilities

• Supports Social Work activities (transportation, meals, resources, scheduling, and documentation).
• Supports Utilization Management activities (review, reporting, routing, and data entry).
• Supports Case Management activities (home health, post-discharge planning, and coordination).
• Care Management Department support (revenue cycle awareness, clerical coverage, timely emails, and customer service).
