Brightwater Health is seeking a Care Coordination Program liaison in Duluth, MN. The role connects clients with Brightwater Health programs and outside providers to close gaps in care, supporting transitions and referrals while ensuring continuity of services.

Responsibilities include engaging clients, navigating services, documenting in the EHR, and participating in team case consultations. A focus on relationship-building and cultural competency is essential.

Qualifications

• Bachelor's degree in human services or related field with 2,000 hours of mental health experience.
• OR high school diploma with 6,000 hours of mental health experience.

Responsibilities

• Reach out to assigned clients to begin Care Coordination services.
• Help clients navigate internal and external services.
• Coordinate care across Brightwater Health programs and external community resources to support the client's overall needs.
• Coordinate transitions for clients discharging from higher levels of care.
• Provide interim support within approved scope to clients waiting for services.
• Reach out to clients who have disengaged to identify barriers and support re-engagement.
• Track internal and external referrals.
• Maintain accurate, timely documentation in the electronic health record.
• Participate in team meetings, case consultations, and supervision.
