Job summary

AHL HealthCare Group is seeking Bridges Counselors in the Duluth area to advocate, support, educate, and care for individuals with disabilities and mental illness in our Bridges to Success program. Roles are in-home and community-based with flexible schedules and advancement opportunities.

Responsibilities include implementing person-centered plans, travel within the Duluth area, and maintaining professional client relationships.

Qualifications

• Must be at least 18 years old and fluent in English.
• Driver's license with access to a vehicle to transport clients.
• Ability to pass MN DHS background study.
• Mobile device capable of receiving/sending calls and using our app for documentation.

Responsibilities

• Implement person-centered, positive behavior support plans to help independence.
• Meet or exceed direct client time target of 90% with ≤10% indirect time.
• Understand client history, skills, vulnerabilities, support plans and emergency procedures.
• Provide support and supervision per each plan using therapeutic interventions.
• Encourage community engagement within a 10-mile radius of clients' homes.
• Regularly travel in the community to meet clients.
• Build professional relationships with clients and families.
• Maintain a professional appearance for role modeling.
