Job summary

Advantage Behavioral Health Systems is seeking a Clinician/Discharge Planner to provide crisis assessments, psychosocial evaluations, and discharge planning for individuals at the BHCC in Athens, GA. The role involves engaging in an interdisciplinary treatment team, coordinating services, and ensuring seamless transitions to appropriate levels of care.

The position schedule is Wednesday, Thursday, and Friday, 7:00 am to 7:30 pm.

Qualifications

• Master’s degree in a related area from an accredited college.
• GA associate licensure or full clinical licensure in the state.
• Preferred: 1 year of full-time (or equivalent part-time) behavioral health experience.

Responsibilities

• Conduct crisis assessments and psychosocial evaluations.
• Develop discharge plans and coordinate needed services.
• Collaborate with community partners and internal teams for case management.
• Document all assessments and clinical activities according to policy.
• Coordinate transportation and follow-up for discharge.
