Qualifications

• Bachelor's degree in social services, psychology, or related field.
• Two years' experience working with clients with emotional or behavioral challenges.
• Valid driver's license and insurable driving record.

Responsibilities

• Collaborate with outpatient teams to develop Individual Service Plans addressing psychosocial needs.
• Coordinate care across physical and behavioral health, social services, housing, education, and employment.
• Arrange appointments, document outcomes in clinical records, and ensure privacy compliance.
