Job summary

University Health seeks a Senior Social Work Manager to lead a multidisciplinary care team in an inpatient setting in San Antonio, TX. You will oversee care coordination, ensure payer requirements are met, and promote patient-centered planning from admission through discharge.

Ideal candidates hold a Master’s in Social Work, have 3+ years of hospital inpatient experience, and at least 1 year in management or senior social work roles. CCM or ANCC certification is desirable.

Qualifications

• Minimum of one year recent management or senior social worker experience.
• 3 years of hospital (inpatient) experience.
• 2 years inpatient case management or utilization management preferred.

Works with a multi-disciplinary team to ensure a patient-centric, individualized treatment plan of care for patients. Facilitates the delivery of services, evaluates effectiveness, tracks outcomes, and functions as the manager of the team.

EDUCATION/EXPERIENCE

A master’s degree in social work is required. A minimum of one year recent management or senior social worker is required with 3 years of hospital (inpatient) experience. 2 years of inpatient case management, or utilization management is preferred.

LICENSURE/ CERTIFICATIONS

Certification in Case Management or Care Coordination and Transition Management is highly desirable (CCM or ANCC).
