Job summary

Amedisys is seeking a Patient Services Coordinator to support scheduling and coordination of patient visits, Oasis assessments, and clinician allocations. You will work closely with field staff to ensure timely care delivery in home health and hospice settings.

Responsibilities include managing calls, coordinating transfers, and ensuring proper documentation and reporting. A strong background in healthcare scheduling and administration is preferred, with a commitment to patient-centered service.

Qualifications

• Must have high school diploma or GED.
• One year of scheduling experience in a healthcare environment preferred.
• Administrative experience in healthcare settings is a plus.

Responsibilities

• Ensure patient assessment visits and Oasis visits are scheduled and performed on time.
• Assign and schedule clinicians efficiently by geography and care requirements.
• Coordinate with field clinicians to complete visits as ordered and within centers.
• Handle calls from patients and staff regarding scheduling issues.
• Refer clinical or performance concerns to the clinical manager.
• Monitor hospitalized patients and ensure timely follow-up by the team.
• Communicate with patients, clinicians, referrals, and staff to maintain timely flow.
• Generate reports and act on workflow insights as needed.
• Support transfers of patients between care centers and hospice services.
• Maintain supply closet and liaise with field for patient supply needs.
• Perform other duties as assigned.
