Job summary

The role conducts patient assessments, coordinates care transitions, and educates patients and families about services while collaborating with physicians, nurses, and post-acute providers. The ideal candidate will have RN licensure with nursing degrees as listed, strong organizational and communication skills, and the ability to manage discharge planning for diverse age groups.

Qualifications

• Excellent organizational and communication (written and verbal) skills expected.
• Knowledge of case management practices and care transitions.
• Ability to collaborate with physicians, nurses, social workers and home health agencies.
• Strong documentation, discharge planning and transition of care abilities.

Responsibilities

• Conducts patient assessments to determine needs and barriers to disease management or recovery.
• Implements interventions to address identified needs and regulatory requirements.
• Monitors effectiveness of care plans and intervenes to address progression delays.
• Arranges appropriate post-acute services and procurement of durable medical equipment.
• Coordinates transition of care as indicated and assesses discharge needs across age groups.
