Qualifications

• Associates in Nursing required; Bachelor's in Nursing required; Master's in Nursing not required.
• Strong knowledge of pediatric, adolescent, adult and geriatric care.
• Experience in care coordination and patient education.

Responsabilités

• Conducts patient assessments to determine needs and barriers to disease management.
• Implements interventions and monitors care plan effectiveness.
• Coordinates transitions of care to post-acute services and Arranges post-acute services/DME as needed.
• Documents assessment findings and education provided in clinical records.
• Educates patients and families about processes and services.
• Participates in performance improvement projects and committees.
