Job summary

Access Community Health Network in Illinois is seeking a Care Coordinator who will guide patients through health care transitions, provide education and advocacy, and support self-management. The role leads intake, builds care plans, maintains patient panels, and coordinates care across outpatient to inpatient settings, with bilingual Spanish a plus and EMR experience.

Qualifications

• Bachelor’s Degree required; Master’s Degree preferred.
• Current, unrestricted Illinois license: LCPC, LPC, LCSW or LSW required.
• Minimum two years of experience in a clinical setting.

Responsibilities

• Serve as team lead for intake process, development and maintenance of care plan and promoting health programs and services.
• Utilize information systems and decision support; manage a patient panel to educate and track patients by disease, risk, and self-management status.
• Lead Patient-Centered Care Planning including assessment, plan, implementation/intervention and evaluation.
