Job summary

Wellstar Health System is hiring an RN Care Coordinator for Cobb Hospital. The role focuses on assessing transitional care needs, coordinating across the care continuum, and engaging with patients and families to ensure care goals are met. It requires hospital experience and an active Georgia RN license.

You will work full-time days onsite, collaborating with physicians and an interdisciplinary team to facilitate discharge planning and optimize resource use.

Qualifications

• Associate's Degree in Nursing required and Georgia RN license.
• Minimum 1 year nursing experience in acute care setting.
• Excellent written and verbal communication and problem-solving skills.

Responsibilities

• Assess transitional care needs and coordinate across the care continuum.
• Engage with patient and family to ensure care needs are met and discharge planning.
• Collaborate with physicians, care team, and payers to optimize care progression.
• Document thoroughly in the electronic medical record and track progress.
• Participate in interdisciplinary rounds and post-acute referrals as needed.
