Job summary

Mass General Brigham Home Care, Inc. is seeking a Registered Nurse to join our home care team for part-time weekend coverage in the Boston area. You will lead patient care plans, deliver 1:1 skilled nursing, and work with an interdisciplinary team to manage complex conditions in patients' homes.

The role emphasizes autonomy, compassionate patient relationships, and top-of-license practice, with travel within the service area and the use of handheld devices for charting and coordination.

Qualifications

• Graduate of an approved School of Nursing.
• Current Massachusetts license as Registered Nurse.
• Minimum of one year previous nursing experience in acute care setting; home care preferred.
• Travel within MGB Healthcare at Home geographic area.
• Ability to use computer software and hand-held patient care devices.
• Strong interpersonal and customer service skills; communicate effectively in writing and verbally.

Responsibilities

• Lead and coordinate individualized patient care plans.
• Deliver 1:1 skilled nursing care in the home.
• Collaborate with interdisciplinary clinicians for comprehensive care.
• Provide advanced clinical treatments and patient education.
• Support transitions and prevent hospital readmissions.
• Complete starts of care, resumptions of care, evaluations, or revisits.
