Job summary

Catholic Charities New Hampshire is seeking a Registered Nurse to join our Nurse Care Coordinator team on a per diem basis. The role focuses on ensuring patients access care through the HCH program, addressing medical and behavioral health needs, and supporting MAT participants.

The RN will perform comprehensive assessments, develop care plans with the interdisciplinary team, and coordinate referrals while maintaining EMR documentation.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed by the state of New Hampshire as a Registered Nurse (RN) and/or compact license.
• CPR/BLS certification required.
• Minimum of 3 years of professional nursing experience.
• Valid Driver’s License or ability to transport to off-site locations.
• Experience with substance use disorders and case management preferred.

Responsibilities

• Perform triage and rapid clinical assessments for patients at shelter-based clinics in partnership with the HCH behavioral health team.
• Initiate emergency response in life-threatening situations and notify HCH providers.
• Assess and document vital signs, symptoms, and chief complaints for clinic visits.
• Conduct comprehensive bio-psycho-social assessments and SUD screenings using evidence-based tools.
• Develop individualized care plans and provide health education and counseling.
• Refer patients to appropriate care levels and communicate findings to providers.
• Ensure EMR is up to date with assessments, results, and consults.
• Coordinate all aspects of care to ensure continuity within the HCH team.
• Arrange referrals and transportation support for diagnostics and specialty care.
• Support nursing interventions for medication administration and point-of-care testing.
