You will assess learning needs, educate patients, reconcile medications, schedule PCP follow‑ups, document encounters in the EHR, and maintain HIPAA compliance while providing compassionate, culturally competent care.

Qualifications

• Active Registered Nurse license in Arizona
• Current Basic Life Support (BLS) certification
• Level I Fingerprint Clearance Card or ability to obtain within 7 working days
• Reliable transportation for organizational reassignment
• 1 year of hands-on patient care experience (minimum)

Responsibilities

• Communicate with recently hospitalized patients by telephone as advised by providers
• Collaborate with medical providers and interdisciplinary team to optimize patient health
• Document patient encounters in EHR/EMR systems
• Monitor messages and tasks in EHR/EMR to ensure timely responses
• Schedule PCP follow-up appointments post-discharge and coordinate care transitions
