Job summary

Qualifications

• Advanced nursing skills and knowledge of resource management.
• Experience with discharge planning and care coordination.
• Ability to coordinate across Medical, Nursing and Ancillary departments.

Responsibilities

• Coordinate clinical care for designated patient population.
• Lead discharge planning and transition of care.
• Collaborate with health care team to improve LOS and patient satisfaction.
• Coordinate home care services and equipment needs.
• Facilitate timely tests, labs and results communication.
