Job summary

Live Well Homecare is seeking a qualified Care Coordinator RN (CC RN) to assess transitional care needs and coordinate discharge planning across the care continuum. You will engage patients and families to ensure care plans are met and optimize outcomes.

The role requires acute care experience, RN license, BLS certification, and strong communication and organizational skills to navigate interdisciplinary teams and documentation in a fast-paced environment.

Qualifications

• RN license (state or multi-state) required.
• BLS certification required.
• Minimum 1 year of acute care nursing experience.
• Excellent communication and organizational skills.

Responsibilities

• Assess psychosocial and functional needs of patients.
• Coordinate discharge planning with interdisciplinary teams.
• Facilitate patient and family education about care plans.
• Address barriers to timely discharge and coordinate post-acute referrals.
• Maintain accurate documentation of assessments, care progression, and discharge plans.
