Job summary

Harborview Medical Center in Seattle is seeking a Registered Nurse to join our Care Management Team on the day shift at 80% FTE. You will coordinate inpatient care and support timely transitions for patients across care levels.

This role requires two years of inpatient nursing, case management experience, WA RN license with BLS. You will liaise with patients, families, medical teams and social services to ensure safe discharges.

Qualifications

• Two years of recent inpatient nursing experience in trauma/general surgery, critical or acute care.
• One year of case management or discharge planning experience.
• Current license to practice as a registered nurse in the State of Washington.
• Current BLS certification.

Responsibilities

• Coordinate inpatient care with care transitions and discharge planning.
• Serve as liaison between patients, families, medical teams, nursing staff and social services.
• Support transitions of care from admission through discharge.
