Job summary

Christ Community Health Services in Jackson, TN is hiring a Referral Coordinator to support our care teams by coordinating referrals, obtaining records, and ensuring complete, timely information for patients with complex health needs.

You will route documents, track status, liaise with hospitals and insurers, arrange external services, and maintain accurate referral data while upholding our faith‑based, compassionate care model.

Qualifications

• Associate degree required; healthcare setting preferred.
• 2–4 years of experience in a healthcare setting.
• Experience with electronic medical records (EMR) preferred.
• Strong communication and organizational skills; ability to collaborate with diverse teams.

Responsibilities

• Route appropriate incoming faxes to the correct clinic daily for patient visits.
• Follow up to ensure faxed data is processed to keep patient care uninterrupted.
• Read consult notes to determine status (critical, normal, inconclusive).
• Retrieve medical and dental records and transfer or fax as requested.
• Identify resources to meet patient needs and schedule referrals for external testing or services.
• Obtain insurance authorization and liaise between patients and insurers for prior authorizations.
• Provide patients with information about external providers to coordinate care.
• Communicate with patients, hospitals, insurers, and providers to coordinate care and complete services.
• Complete follow-up on incomplete referrals and reschedule or request reports as needed.
• Maintain updated directory of community resources for patient needs.
• Facilitate communication between patients, providers, and clinical staff regarding referral status.
