Job summary

Qualifications

• High School Diploma or GED required.
• 1 year outpatient, ambulatory, specialty, or community healthcare experience required.
• 2 years outpatient clinical setting with PCPs experience preferred.

Responsibilities

• Coach and guide patients and caregivers to address key health tasks and self-management.
• Coordinate care across the care team to align goals and track progress.
• Conduct home visits and follow-up to support transitions across care settings.
• Assist with medication reconciliation and post-discharge planning.
• Communicate with patients, families, and clinicians to ensure safe handoffs and coordinated care.

Education
