This role collaborates with the care team to process transfer documentation, respond to inquiries, support the Medicare Appeals process, and help coordinate patient transportation and communications with leadership.

Qualifications

Responsibilities

• Provide non-clinical administrative support for Hub Transfer Center and Care Coordination team to support timely admissions and transfers.
• Process documentation for patient transfers to post-acute care settings (photocopies, faxes, electronic submissions, transportation paperwork).
• Respond to inquiries from post-acute care providers, patients, and families and direct to appropriate staff.
• Support Medicare Appeal Process.
• Monitor Care Coordination task list and coordinate patient transportation as needed.
• Communicate operational activities to leadership and participate in performance improvement efforts.
• Perform other duties as assigned.
