Job summary

Molina Healthcare seeks a Medical Assistant to support the Care Connections team in Long Beach, CA. The role focuses on patient navigation, follow-up coordination, and administrative clinical support to improve quality and cost-effectiveness of member care.

Responsibilities include coordinating care after visits, communicating results to members and PCPs, assisting with care management referrals, and ensuring timely follow-up and documentation.

Qualifications

• At least 1 year of health care experience in a family practice, home health, primary care, palliative care or hospice setting, or equivalent.
• Graduate of an accredited medical assistant program.
• Understanding of patient care, medical terminology, coding, pharmacology within MA scope.
• Excellent customer service skills.
• Ability to work independently or as part of a team.
• Desire to meet Care Connections metrics and provide excellent service.
• Strong attention to detail, ability to multi-task and meet deadlines.
• Empathy for senior, disabled and vulnerable populations.
• Strong communication skills; proficient with MS Office and able to learn new programs.
• Ability to lift up to 50 lbs.

Responsibilities

• Coordinates care between members and providers after appointments to support navigation and follow-up.
• Clarifies orders with providers and escalates issues when unable to resolve.
• Supports processing and follow-up for care management and PCP referrals for high-risk members.
• Escalates issues related to prior authorizations for medications and DME.
• Communicates lab results and exam findings to members and PCPs.
• Communicates with nurse practitioners on member health issues for timely follow-up.
• Sends electronic messages/faxes to PCPs to support care coordination.
• Closes gaps in health care by preparing member lab result letters and coordinating follow-up.
• Fulfills nurse practitioner supply orders and maintains inventory.
• Performs administrative tasks such as phone/email response and updating member records.
• Facilitates follow-up calls to ensure positive member experience.
• Assists with special projects under clinical leadership.
