Job summary

Molina Healthcare, Inc. is seeking a Care Manager to support LTSS coordination and care management activities.

You will collaborate with a multidisciplinary team to coordinate member care across settings and ensure progress toward outcomes. Responsibilities include conducting comprehensive member assessments, facilitating waiver enrollment, developing care plans, monitoring progress, and promoting integration of services.

Qualifications

• 2+ years of health care experience, including LTSS and care management.
• Clinical licensure and/or certification required only if required by state contract, regulation, or licensing mandates.
• Valid driver’s license and reliable transportation for job travel.
• Ability to work independently and with diverse populations.
• Familiarity with community resources and behavioral health integration.

Responsibilities

• Completes comprehensive member assessments within timelines, including in-person home visits as required.
• Facilitates comprehensive waiver enrollment and disenrollment processes.
• Develops and implements care plans in collaboration with members, caregivers, physicians and the care team.
• Monitors care plan effectiveness, documents interventions/outcomes, and adjusts as needed.
• Promotes integration of services across behavioral health and LTSS with home/community resources.
• Assesses medical necessity and authorizes waiver services.
• Facilitates interdisciplinary care team meetings for approval/denial of services.
