Job summary

David Raines Community Health Center in Shreveport, LA is seeking a Care Coordination Manager to lead patient care coordination, supervise staff, and liaise with insurers and providers. The role focuses on closing care gaps, scheduling, and improving outcomes across the clinic network.

The position requires strong communication, scheduling experience, and the ability to manage dashboards and referrals while delivering excellent patient service.

Qualifications

• High School Diploma or GED is required or equivalent (Associate degree preferred).
• At least 5 years of experience in patient scheduling or healthcare setting.
• Must communicate effectively, orally and in writing, with physicians, staff and patients.
• Computer literacy is necessary (Word, Excel, Outlook).
• Must be available to work after hours and weekends as required by management.

Responsibilities

• Supervise staff engaged in patient care coordination, including appointment setting, reminder calls, care gaps, and assessments.
• Identify and prioritize patients with care gaps and communicate actions to leadership and providers.
• Follow up on actions and outcomes related to care gaps.
• Maintain tracking/reporting methods for care gaps, referrals, and outcomes.
• Develop and utilize dashboards and scorecards to monitor care coordination results.
• Support the business through excellent customer service and relationships with referral providers.
• Coordinate referrals and outreach to ensure timely contact for appointments after discharge.
• Monitor performance measures related to patient care coordination and care gaps.
• Identify improvement opportunities and collaborate with leadership on clinical outcomes.
• All other duties as required.
