Qualifications

• 2+ years of health care experience, preferably in care management or related settings.
• Bachelor's degree may be required by state contract or regulations.
• Valid driver’s license and reliable transportation for travel requirements.
• Knowledge of community resources.
• Strong time-management and prioritization skills.
• Ability to work independently and with minimal supervision.

Responsabilités

• Completes member assessments per timelines to identify care coordination eligibility.
• Develops and implements care plans with member and care team.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness and documents progress.
• Maintains ongoing member caseload and outreach.
• Promotes integration of behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings.
• Addresses barriers to care and coordinates with leadership as needed.
• Travel may be required 25-40% depending on state requirements.
