Job summary

Molina Healthcare, Inc. is seeking a Care Coordinator to support care management and care coordination across the continuum. You will collaborate with a multidisciplinary team to ensure member progress toward outcomes and align efforts with strategy for high-quality, cost-effective care.

Responsibilities include assessments, care plan development, visits, monitoring, and facilitating ICT collaboration while promoting integration of services and resources for members.

Qualifications

• 2+ years in health care, preferably care management or related setting.
• Clinical licensure and/or certification required only if state contract/ regulation requires it.
• Valid driver's license, reliable transportation for travel.
• Demonstrated knowledge of community resources.
• Ability to work independently with minimal supervision and self-motivation.
• Excellent time-management and prioritization skills.
• Strong verbal and written communication skills.
• In some states, a bachelor's degree in a health care related field may be required.

Responsibilities

• Completes assessments of members per regulated timelines and identifies who qualifies for care coordination.
• Develops and implements care plans with the member and care team to address needs and goals.
• Conducts telephonic, in-person, or home visits as required.
• Monitors care plan effectiveness, documents interventions, and suggests changes.
• Maintains ongoing member caseload for regular outreach and management.
• Promotes integration of services across behavioral health, LTSS, and community resources.
• Facilitates interdisciplinary care team meetings and collaboration.
• Uses motivational interviewing and Molina clinical guideposts to educate and motivate change.
• Assesses barriers to care and provides coordination assistance as needed.
• Collaborates with licensed care managers and leadership as required.
• Travel 25-40% may be required depending on state/ contract requirements.
