Molina Healthcare, Inc. seeks a Care Coordinator to support care management activities and collaborate with a multidisciplinary team to coordinate integrated member care across the care continuum.

You will help progress toward outcomes and contribute to strategies that enhance quality and cost-effective care. You will conduct assessments, develop care plans with members and providers, perform home visits as needed, monitor progress, and promote integration of services including behavioral health

Qualifications

• At least 2 years of health care experience, preferably in care management.
• Licensure/certification required only if state contract requires it.
• Valid driver's license and reliable transportation for travel.
• Knowledge of community resources and local services.

Responsabilités

• Assess members per timelines to determine eligibility for care coordination.
• Develop and implement care plans with member and care network.
• Conduct telephonic, in-person, or home visits as required.
• Monitor care plans and document outcomes, adjusting as needed.
• Maintain ongoing member caseload and outreach.
• Promote integration of behavioral health, LTSS, and home/community resources.
• Facilitate ICT meetings and collaboration.
• Use motivational interviewing and Molina clinical guidelines.
• Identify barriers to care and provide coordination support.
• Collaborate with licensed care managers as needed.
• Travel 25-40% depending on state requirements.
