Job summary

Peerstar LLC is seeking an experienced Care Coordinator to join our Operations Team. The role focuses on coordinating services, scheduling staffing, and ensuring continuity of care across Medicaid-funded home care programs.

Ideal candidates have 1–3 years in care coordination or related roles, with strong organizational and communication skills. Experience in home care and Medicaid programs is highly valued.

Qualifications

• 1-3 years of experience in Care Coordination, Home Care Scheduling, Healthcare Staffing, Client Services, Case Management Support, or Healthcare Operations.
• Experience in Home Care Agency, Home Health Agency, Personal Care Agency, Behavioral Health Program, or Medicaid-funded program is preferred.
• Experience coordinating schedules, managing service hours, or staffing field-based employees is preferred.
• Strong organizational, communication, and problem-solving skills.
• Ability to prioritize multiple tasks in a fast-paced environment.
• Proficiency with Microsoft Office and HIPAA-compliant documentation.

Responsibilities

• Staff peers with Certified Peer Specialists based on IRP authorization, availability, and geographic needs.
• Monitor utilization to ensure authorized hours are maximized.
• Identify and resolve service gaps or underutilization trends.
• Conduct outreach to support peer engagement and retention.
• Collaborate with CPS and leadership to maintain consistent service delivery.
• Track scheduling trends and recommend operational improvements.
• Maintain compliant documentation and follow HIPAA standards.
