Job summary

Qualifications

• High school education or GED required.
• Associate’s/Technical degree preferred or equivalent experience.
• Three years of relevant healthcare experience preferred.

Responsibilities

• Handles and coordinates incoming calls related to patients, physicians, referrals and agency services regarding physician orders, patient questions, and patient referrals.
• Facilitates telephone communications between nursing, therapy, aide, social services, physicians, etc. regarding changes in patient/staff schedule, test results, etc.
• Assists with staffing/scheduling activities, soliciting input from managers.
• Provides follow-up for hospitalized patients, ensuring subsequent agency referral.
