Job summary

Adventist Health Clear Lake is seeking an experienced Home Health Intake Coordinator to process referrals, coordinate patient admissions, and work with physicians and clinical teams. You will verify benefits, maintain patient records, answer calls, and coordinate referrals with clinical staff.

Qualified candidates will hold an active RN/LVN/LPN license, CPR certification, and Epic experience, with at least 1.5 years in a similar role and an Associate degree in Nursing or equivalent.

Schedule: Monday Friday, Day Shift

Working Hours: 8:00 AM 4:30 PM
