Columbia River Mental Health Services is seeking a Care Coordinator Lead to support the Health Homes Department in SW Washington. You will coordinate care for individuals with chronic health conditions, provide outreach to community partners, and lead a team to improve health outcomes while ensuring cost-effective care.

Responsibilities include supervising staff, monitoring program goals, data collection for reporting and billing, and direct client service for a large portion of your time.

Qualifications

• Approximately 1-2 years of experience in social services with mental illness or chemical dependency preferred.
• Bachelor's degree in Social Work or related field preferred.

Responsabilités

• Lead and train care coordination team.
• Provide regular updates to management on coaching.
• Direct client services at least 75% of time.
• Monitor project goals and outcomes monthly.
• Collect and manage data for reporting and billing.
• Coordinate outreach with community partners.
• Assist participants with navigating the healthcare system.
• Coordinate same-day services and transportation arrangements.
