Job summary

Pro Care by Abound Health in Southfield, MI is seeking a compassionate Care Coordinator to support individuals with Michigan Waiver Services. You will build relationships, coordinate services to help clients live more independently, and supervise direct care staff reporting to the Program Director.

The role requires strong communication, organization, and problem-solving, with the ability to manage multiple priorities in a fast-paced environment.

Qualifications

• High school diploma or GED required.
• Experience in care coordination, case management, social services, healthcare, or a related field.
• Strong communication, organization, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of IPOS implementation and Michigan Waiver Services is preferred.
• Proficiency in Microsoft Office; OnTarget experience is a plus.
• Valid driver’s license, reliable transportation, and ability to pass background checks.

Responsibilities

• Build trusted relationships with clients, families, direct care staff, and community partners.
• Coordinate individualized services that support each client’s goals, independence, and quality of life.
• Supervise, coach, and support direct care staff.
• Develop client goals and interventions and maintain them in OnTarget.
• Coordinate staff schedules and backup coverage to provide consistent, reliable support.
• Connect clients and families with helpful community services and resources.
• Maintain accurate records and ensure services meet Michigan waiver, IPOS, and Pro Care requirements.
