Job summary

Careforth, Inc. is seeking a Manager, Care Management in Michigan to lead a team of clinical and non-clinical care managers and to deliver high quality, home-based care. The role combines hands-on duties with team leadership to ensure services meet regulatory, contractual, and quality standards.

You will guide assessments, care planning, and discharge planning while maintaining strong relationships with external partners and ensuring program improvements based on data-driven insights.

Qualifications

• Bachelor’s degree or equivalent in health care or social services required; 7+ years in healthcare case management.
• Active RN license required; state-specific qualifications may apply.
• Must reside in Michigan and be within driving distance to Detroit office; willingness to travel.

Responsibilities

• Lead and manage a team of clinical and non-clinical care managers to engage, motivate, and educate family caregivers to keep care at home.
• Maintain working manager responsibilities including direct caregiver support and operational follow-through.
• Oversee care management functions for policy, procedures, and regulatory alignment (NCQA, contracts).
• Coach and develop staff to meet department goals and KPIs in a high-performance environment.
• Plan, organize, and oversee timely completion of assessments, care plans, and discharge planning.
• Interpret policies and ensure proper application across teams and contractual requirements.
• Track outcomes, generate program reports weekly/monthly, and drive quality improvements.
• Review incidents and monitor use of person-centered care plans; liaise with external partners to improve collaboration.
