Job summary

Pro Care by Abound Health in Southfield, MI is seeking a compassionate Care Coordinator to support individuals with Michigan Waiver Services. You will build relationships, coordinate services to help clients live more independently, and supervise direct care staff reporting to the Program Director.

The role requires strong communication, organization, and problem-solving, with the ability to manage multiple priorities in a fast-paced environment.

Qualifications

• High school diploma or GED required.
• Experience in care coordination, case management, social services, healthcare, or a related field.
• Strong communication, organization, and problem-solving skills.
• Ability to manage multiple priorities in a fast-paced environment.
• Knowledge of IPOS implementation and Michigan Waiver Services is preferred.
• Proficiency in Microsoft Office; OnTarget experience is a plus.
• Valid driver’s license, reliable transportation, and ability to pass background checks.

Reporting to the Program Director, you will help individuals receiving Michigan Waiver Services live more independently and remain connected to the support they need. Base pay is determined by factors such as work location, experience, job-related skills, and relevant education or training. In addition to base pay, total compensation may include various other forms of pay.
