Job summary

ABI Resources is seeking a Care Management Coordinator to help individuals access long-term care services at home. You will collaborate with formal and informal caregivers to ensure clients receive the support needed to stay independent and well.

You will coordinate with the MFP Unit, handle admissions/discharges, assist with forms, triage inquiries, and monitor compliance with deadlines. You will mentor new administrative staff and support the team as required.

Responsibilities

• Collaborate with the MFP Unit to coordinate person-centered services and monitor client progress.
• Manage administrative tasks related to hospital admissions and discharges, including records.
• Assist with completion of applications, forms, and documentation, ensuring timely submission.
• Triaged client, family, and provider inquiries and follow up as needed.
• Support authorization requests, service orders, and client monitoring for the MFP Unit.
• Monitor compliance with deadlines and procedures and coordinate assessments.
• Facilitate referrals to community agencies and resources.
• Provide administrative support to team members and mentor new staff.
