Job summary

NYU Langone Hospital—Suffolk in eastern Long Island is hiring a Care Management Assistant—Full Time, Days. Working under the RN Care Manager and LCSW, you will help set up post-hospital care and support the discharge planning process.

You will assist with intake, tracking, and communication of reviews to payors, maintain case files, update referrals, and verify health insurance details to smooth transitions.

Qualifications

• High School Diploma or GED is required.
• Knowledge of medical terminology is required.
• Proficiency with Word, Excel and email.
• Excellent interpersonal skills to communicate with internal and external customers.
• Strong organizational and analytical skills to manage multiple tasks.
• Ability to work in a team and independently.
• Good oral and written communication skills.

Responsibilities

• Participates in discharge planning and coordinating post-acute care referrals.
• Facilitates timely transmission of reviews to payors and communicates with care managers; documents denials and supports appeals.
