Baptist Health System, Inc. is seeking a Care Coordination Assistant for its Care Management department at Baptist South. This full-time, daytime role involves clerical tasks and moving around the hospital to obtain signatures and support RN Case Managers and Social Workers with patient needs.

Responsibilities include coordinating care across the continuum and providing patient-centered care during transitions. Requires 1–2 years hospital experience, high school diploma, and BLS.

Competenze

• 1-2 years hospital experience preferred.
• Basic Life Support (BLS) required.
• High School Diploma required. Licenses/certifications preferred include CCMA, CMA, RMA or NCMA.

Mansioni

• Provide clinical and clerical support to the Care Coordination Team under supervision.
• Coordinate care across inpatient, outpatient, post-acute and community settings to ensure appropriate use of resources.
• Assist RN Case Managers and Social Workers with patient care transitions and needed tasks.
