Memorial Health University Medical Center in Savannah, GA is seeking a Case Management Assistant to coordinate with the RN case manager and social worker, provide clerical support, and help expedite discharges. You will monitor PAC referrals, relay status to the care team, and assist with Medicare communications and discharge planning in a busy hospital setting.

Qualifications

• High School Graduate / GED.
• No experience required.

Responsibilities

• Coordinates with RN case manager and social worker for support needed.
• Communicates updates on progress throughout the day.
• Refers patient questions back to the case manager/social worker.
• Provides clerical support in multidisciplinary rounds and updates the PAC referral system as directed.
• Monitors PAC referrals and intervenes to expedite discharge.
• Communicates results of PAC referrals with the patient, including the Patient Choice letter.
• Delivers the Important Medicare Message (IMM) to eligible patients per policy.
